Notice required under the federal No Surprises Act
Effective Date: October 1, 2026
If you don't have insurance, or you have insurance but are paying for your care yourself, you have the right to receive a Good Faith Estimate of what your care is expected to cost before you receive it.
A Good Faith Estimate is a written, itemized breakdown of the services you can reasonably expect to receive and what each is expected to cost. It is not a bill, and your actual charges may differ.
This right applies to you if:
Visits billed to Medicare follow Medicare's own rules and are not covered by this notice.
If your actual bill is $400 or more above your Good Faith Estimate, you have the right to dispute it. You can:
There is a $25 non-refundable fee to file a federal dispute. If the dispute is decided in your favor, the $25 is deducted from what you owe. An independent reviewer, not Vodic PT, decides the outcome.
Keep your estimate. Save a copy or take a picture of it. You will need it if you ever dispute a bill.
